Your health matters. This page helps guide our Medicare-eligible patients through the process of aligning with a primary clinician so that care can be better coordinated, supported, and personalized.

Why Alignment Matters

Being aligned with a primary clinician allows your care team to access important information and support your health journey more effectively. Choosing a provider does not affect your Medicare Part A or Part B benefits, nor limits your ability to see other doctors. It simply helps ensure smoother coordination of your overall care.

How to Select Your Primary Clinician

If you haven’t yet chosen us as your primary clinician through Medicare.gov, here are the steps:

1. Visit Medicare.gov and log in.
2. On the homepage, select Providers & Services, then Find care providers.
3. Choose Doctors & clinicians.
4. Search by ZIP code, city, street, or clinician’s name or specialty.
5. In the results, locate your clinician and click their name. Select Save to Favorites.
6. In the pop-up, check Make this my primary health care provider in my favorites, select the correct office location if applicable, then click Save to Favorites.
7. Confirm the update by reviewing My Favorite Providers.
